By accessing or using MIA Health services, including our website, mobile application, and telehealth platform, you agree to be bound by these Terms of Service.
If you do not agree to these terms, please do not use our services. We reserve the right to update these terms at any time, and your continued use constitutes acceptance of any changes.
MIA Health provides telehealth consultations, prescription management, IV therapy services, and wellness programs through licensed healthcare providers.
Our services are available to individuals 18 years of age or older and are subject to medical eligibility as determined by our providers. Services may vary by location and availability.
You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account.
You agree to provide accurate, current, and complete information during registration and to update it as necessary. We reserve the right to suspend or terminate accounts that violate these terms.
Pricing for services is displayed at the time of purchase and may be subject to change. Payment is processed securely through our third-party payment processor.
Refunds are handled in accordance with our Payment Terms & Refund Policy. Subscription services may be cancelled at any time before the next billing cycle.
MIA Health and its affiliates shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services.
Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim. This limitation applies to the fullest extent permitted by law.
We may revise these Terms of Service at any time by posting the updated version on our website. Material changes will be communicated via email or an in-app notification.
Your continued use of our services following the posting of revised terms constitutes your acceptance of the changes. We encourage you to review these terms periodically.
Treatments classified as "Rx" in our catalog — including peptide therapies (Sermorelin, Ipamorelin, GHK-Cu and similar), GLP-1 medications (compounded Semaglutide, compounded Tirzepatide), sexual wellness Rx (Tadalafil, Sildenafil), and certain IV protocols — cannot be purchased without a valid prescription issued by a licensed healthcare provider following a medical evaluation conducted through the MIA Health platform.
Submitting an intake form, completing payment authorization, or contacting MIA Health does not guarantee that a prescription will be issued. The provider may decline to prescribe, request additional information, or recommend an alternative treatment when clinically appropriate. If a prescription is not issued, any payment authorization placed at checkout is voided automatically.
For full details on the telehealth evaluation process, see our Telehealth Informed Consent. For the FDA disclosure on compounded medications, see our Medical & Educational Disclaimer.
Telehealth services are available only to patients physically located in U.S. states where our providers are licensed at the time of consultation. We do not currently serve patients located outside the United States, in U.S. territories, or in states without an active provider license. The list of eligible states is presented at the start of the intake flow and is updated as our provider network grows.
Several peptide and wellness therapies offered through MIA Health are prescribed off-label at the provider's clinical discretion — meaning the FDA has not specifically approved that compound for the indication being treated. Off-label prescribing is legal and common in U.S. medical practice; however, you acknowledge that off-label therapies have not been evaluated by the FDA for the specific use discussed.
If at any point our catalog includes therapies classified as DEA-scheduled controlled substances, those will be subject to additional federal requirements including the Ryan Haight Act (or its successor regulations) and may require an in-person evaluation before a prescription can be issued. The eligibility check at the start of every intake will surface those requirements when applicable.
You agree to disclose all current medications, medical conditions, allergies, surgeries, family history, and pregnancy status truthfully when completing any intake form. Withholding or misrepresenting medical information may cause your provider to issue a treatment that is unsafe for you, and you accept full responsibility for any harm arising from incomplete or inaccurate information that you provided.
MIA Health reserves the right to deny prescriptions, cancel orders, or terminate accounts when intake information is materially false. In the event of an order cancellation due to false intake information, refunds are issued in accordance with our Payment Terms & Refund Policy.
