SMS Program Documentation
Family Rehab & Research Inc (operating as MIA Health) sends transactional text messages to patients who explicitly opt in: appointment confirmations, reminders and cancellations, order payment confirmations, provider approval notifications, order status updates, and mobile-IV nurse dispatch updates. This page documents the consent flow for messaging compliance review.
During account creation, patients are presented with this optional checkbox — unchecked by default and never required to access our services:
I agree to receive recurring automated SMS appointment and order notifications from MIA Health at the mobile number provided. Consent is not a condition of purchase. Message frequency varies (typically 1–5 per appointment or order). Msg & data rates may apply. Reply STOP to cancel, HELP for help.

1. Patient portal entry — app.miahealthclinic.com/login
The patient chooses “Continue with Email” and then “Register here” to create an account.

2. Create Account form with the SMS consent checkbox
The form shows two separate checkboxes: a required legal acknowledgment (Terms of Service, Privacy Policy, HIPAA Notice, Telehealth Consent) and the optional SMS consent checkbox documented above. Checking the SMS box reveals a required mobile-number field— the number the notifications will be sent to. Creating the account with the SMS box checked records the patient’s opt-in together with that number; leaving it unchecked has no effect on their access to care.
