MIA Health is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this notice of our legal duties and privacy practices.
We may use and disclose your PHI for treatment purposes, including coordinating care with other healthcare providers, sending prescriptions to pharmacies, and communicating with you about your treatment plan.
We may also use your PHI for payment activities such as billing, claims management, and collections, as well as for healthcare operations including quality improvement and compliance auditing.
You have the right to inspect and obtain a copy of your PHI, request amendments to your records, receive an accounting of certain disclosures, and request restrictions on how your information is used.
You also have the right to request confidential communications and to receive a paper copy of this notice upon request. To exercise any of these rights, please contact our Privacy Officer.
We are required by law to maintain the privacy and security of your PHI, notify you promptly if a breach occurs that may compromise your information, and abide by the terms of this notice.
We will not use or disclose your PHI without your written authorization except as described in this notice or as permitted by law.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect your care or access to our services.
